Application for First-time Registration as a Pharmacist Forms
(Please note that these forms are only applicable to National Graduates)
The following forms are available to download for those candidates who were successful in the Professional Registration Examination that was held on Monday, 2 November 2009. The deadline for submission of the application forms is Monday, 7 December 2009, for those who wish to be registered for Wednesday, 16 December 2009.

Form for Submission of Photograph
From 2010 onwards, part of the registration requirements for pharmacists and pharmaceutical assistants is the provision of two recent passport photographs to the PSI. If photographs have not been supplied to the PSI already, they must be enclosed with this form and returned to the PSI immediately. Certificates for continued registration cannot be issued without a photograph. Click here to download the form.


Form for Cancellation of Pharmacist/Pharmaceutical Assistant Registration

Form for Cancellation of Retail Pharmacy Business Registration
This form must be filled in and returned to the PSI to request the cancellation of a current retail pharmacy business registration. Click here to download the form.

Nomination Forms - Superintendent Pharmacist & Supervising Pharmacist
Part of the registration requirements for retail pharmacy businesses is the nomination of a Superintendent Pharmacist and Supervising Pharmacist. When the holder of either of these roles changes, the appropriate Nomination form must be completed and returned to the PSI immediately, along with the associated fee.

Please note that in the case of ownership of a Retail Pharmacy Business by a limited company, the Corporate Body Declaration Form section of the Superintendent Pharmacist Nomination Form must be completed.
