July 2010 - A hospital prescription pad was reported missing to the Pharmacy Department in The Adelaide & Meath Hospital incorporating The National Children's Hospital (AMNCH) on the 01st July 2010. The serial numbers on the prescription pad are as follows: 960551 - 960600.

May 2010 -
A hospital prescription pad was reported missing to the Pharmacy Department in the Adelaide & Meath Hospital incorporating the National Children's Hospital (AMNCH) on the 07th May 2010. The serial numbers on the prescription pad are as follows: 976501-976550. The matter was reported to the Gardai in Tallaght on the 7th May 2010.

March 2010 -
A hospital prescription pad was reported missing to the Pharmacy Department in The Adelaide & Meath Hospital incorporating The National Children's Hospital on the 12th March 2010. The serial numbers on the prescription pad are as follows: 982801-982850. The matter was reported to the Gardai in Tallaght on the 12th March 2010.

March 2010 -
A hospital prescription pad has been reported missing from the National Maternity Hospital, Holles Street. Prescriptions have been presented on the north and south sides of Dublin city. The serial numbers are as follows: 60530-60550.

December 2009 -
A hospital prescription pad was reported missing from The Adelaide and Meath Hospital (incorporating the National Children's Hospital) on the 28th January 2010. The serial numbers on the prescription pad are as follows: 933451-933500. The matter was also reported to the Gardai on the 28th January 2010.

December 2009 -
A hospital prescription pad was reported missing from the Out-Patient Department, Monaghan General Hospital on the 17th December 2009. The serial numbers on the prescription pad are as follows: 5851-5900. The matter was reported to the Gardai on the 18th December 2009.

November 2009 -
The PSI has information which indicates that forged prescriptions were presented at pharmacies located in the Newbridge (Kildare) area in recent days. In the event that you have any suspicions with respect to a particular prescription, please contact the relevant doctor to verify details.

October 2009 -
The PSI has information which indicates that forged prescriptions were presented at pharmacies located in the North Dublin and Meath areas in recent days. In the event that you have any suspicions with respect to a particular prescription, please contact the relevant doctor to verify details.

October 2009 -
A hospital prescription pad was reported missing to the Pharmacy Department in The Adelaide & Meath Hospital incorporating The National Children's Hospital on the 17th September 2009. The serial numbers on the prescription pad are as follows: 979651 – 979700. The matter was reported to the Gardai in Tallaght on the 12th October 2009.

September 2009
- A hospital prescription pad was reported missing to the Pharmacy Department in The Adelaide & Meath Hospital incorporating The National Children's Hospital on the 17th September 2009. The serial numbers on the prescription pad are as follows: 923251 – 923300. The matter was reported to the Gardai in Tallaght on the 22nd September 2009.

September
2009 - The PSI has been informed by the pharmacy department at Beaumont Hospital that it has received information that at least two forged prescriptions on genuine Beaumont Hospital prescription forms have been presented at local pharmacies in recent days. The serial numbers of the prescription forms were 838593 and 854395. The first of these prescriptions was issued to the Emergency Department. The issue point for the second prescription is not known.
