A hospital prescription pad has been reported missing from the National Maternity Hospital, Holles Street. Prescriptions have been presented on the north and south sides of Dublin city. The serial numbers are as follows: 60530-60550.

The serial numbers on the prescription pad are as follows: 979651 – 979700. The matter was reported to the Gardai in Tallaght on the 12th October 2009.

The PSI has been informed by the pharmacy department at Beaumont Hospital that it has received information that at least two forged prescriptions on genuine Beaumont Hospital prescription forms have been presented at local pharmacies in recent days. The serial numbers of the prescription forms were 838593 and 854395. The first of these prescriptions was issued to the Emergency Department. The issue point for the second prescription is not known.
